Utah CCW Application Checklist
1.	Am I qualified/eligible?
2.	Completed and signed the application form
3.	Signed and stamped by certified BCI instructor
4.	Photcopy of driver license
5.	2X2 Color photograph - passport quality
6.	Completed fingerprint card
7.	$51.00 Check paid to Utah Bureau of Criminal Identification
8.	Affix proper postage on the envelope
*You should have at least 5 items in your envelope when completed. More if you have other attachments such as copy of expunged records of arrests and conviction in adult and juvenile courts. A copy of your naturalization or resident immigrant card is also required for naturalized citizens and immigrant residents respectively. A copy of your passport is acceptable.
